Delivery TechniquesThe more severe the dystocia, the greater the need for additional maneuvers. Delivering of the posterior arm is the safest technique for the infant. Episiotomy is very important.

The McRoberts ManeuverThe McRoberts position, or maneuver, is a good first-line technique. If combined with more than gentle traction, a brachial plexus will occur. It usually requires additional strong suprapubic pressure.

Predisposing Factors for Shoulder Dystocia-Related Birth Injuries: Causation AnalysisThis chapter contradicts the claim that more than half of all brachial plexus injuries are associated with uncomplicated deliveries. Infants are injured much more frequently with macrosomia and operative delivery. Twenty-five percent of infants suffer injury, and 5% to 22% become permanent. Shoulder dystocia was recorded in 97% of 135 cases.

Failure of restitution is an ominous finding. Traction on the head injures the brachial plexus. Contractions are compressive and expulsive force, not stretching.
