描述As there are a number of nuances in terms of how to manage mutation carriers (both with and without a concomitant diagnosis of breast cancer), this text provides a comprehensive, state-of-the art review of this field. It represents a valuable resource for a myriad of clinicians and healthcare personnel who interface with these patients. The text discusses the latest recommendations for genetic counseling and risk assessment, provides a framework for considering reducing risk in mutation carriers who do not present with a concomitant diagnosis of breast cancer, and finally elucidates the many considerations of managing a breast cancer patient with a BRCA mutation. The text presents a multidisciplinary approach gleaning insights from imaging, breast surgery, gynecology, plastic surgery, medical oncology, radiation oncology and psycho-oncology..Managing BRCA Mutation Carriers. will be a useful resource for physicians and healthcare providers from amyriad of disciplines who manage BRCA mutation carriers. All chapters are written by experts in their fields and include the most up to date scientific and clinical information..

Risk Assessment for Breast Cancer,ic testing; and most important, ensure that the patient follows through. With each of these steps, a certain proportion of patients is lost. Mary-Claire King, the human geneticist who discovered the . mutation, once stated that “to identify a woman as a carrier only after she develops cancer is a failure of cancer prevention.” We can do better.

Risk Assessment for Breast Cancer, fected . mutation carriers before they develop breast cancer can reduce their risk and increase survival through increased intensity and frequency of screening, prophylactic surgery, and use of chemopreventive agents. Although the tools for genetic testing have been available for the past 20 years,

Genetic Evaluation for Women at Increased Risk, family history of cancer that may be associated with a hereditary cancer predisposition. The collection of an accurate, cancer-focused family history is the foundation of this risk assessment. Professional organizations have created guidelines outlining when patients should be referred to a provider

Imaging Screening and Surveillance, sitivity of screening MRI in this population is 90–95%, more than twice that of mammography, with a specificity of over 90% after the first round of screening. Ten-year breast cancer-free survival, though likely somewhat lower than that with risk-reducing mastectomy, is at least 90%. The incremental

Risk-Reducing Surgery for , Genetic Mutation Carriers,y has been shown to reduce the risk of breast cancer by 90% or more among high-risk women including . mutation carriers. The decision to undergo prophylactic surgery is a choice that patients make, in conjunction with their multidisciplinary healthcare team which often includes genetic counselors, s
