The Hypoxic Drive, Supplemental Oxygen, and Hypercapniaients with chronic obstructive pulmonary disease (COPD). Does oxygen-induced hypercapnia really occur, or is it is just an urban legend? Are there other plausible mechanisms for the rise in CO. levels often observed during oxygen therapy in CO. retainers?

Do Vigorous Spontaneous Respiratory Efforts Lead to Patient Self-Inflicted Lung Injury (P-SILI )?sing concern that vigorous spontaneous respiratory efforts may also predispose to a similar type of injury, termed patient self-inflicted lung injury (P-SILI). P-SILI may occur during unsupported or assisted breathing. This chapter reviews the mechanisms of causation of P-SILI and the measures that

Prone Ventilation in Acute Respiratory Distress Syndromeosition allows improved ventilation-perfusion matching in most patients. However, the clinical impact of prone ventilation had been contentious until recently. This chapter summarizes the evidence behind the efficacy of prone ventilation in mechanically ventilated patients with ARDS.

Neuromuscular Blocking Agents in Severe Acute Respiratory Distress Syndrome: Benefit or Harm?n during the early stages of ARDS. However, adverse effects can arise from NMB use, including persisting muscle weakness and delayed weaning. This chapter evaluates the pros and cons of the use of NMBs in mechanically ventilated patients with severe ARDS.

Ventilator-Associated Pneumonia : Newer Definitions, Controversies, and Perspectivescare Safety Network (NHSN) of the Center for Disease Control and Prevention (CDC) in the USA proposed new definitions as a surveillance technique for VAP. This chapter reviews outcomes from VAP; methods of prevention, including bundled approaches; and the optimal duration of treatment.

Lung-Protective Ventilation and Hypercapnia : How Much Is Permissible?respiratory distress syndrome (ARDS). Low tidal volume ventilation may often lead to hypercapnia. The clinical impact of permissive hypercapnia has been the subject of debate in ARDS. This chapter evaluates the evidence behind clinical outcomes related to hypercapnia in mechanically ventilated patie

Weaning and Liberation from Mechanical Ventilation: Is My Patient Ready?ar weakness. Assessment of the readiness to wean and early liberation from ventilation are important in optimizing clinical outcomes. This chapter reviews how best to perform spontaneous breathing trials considering the current evidence.
