Signal Validation, (TMI) and Fukushima Daiichi NPP accidents, operator error resulting from incorrect signals represents one of the main contributors to historical severe accidents. During the TMI accident, due to wrong indications, the operating staff did not build up a correct situational awareness about the plant status.
