Arthroplasty,Reconstruction by arthroplasty restores function and mobility to joints incapacitated by pain, deformity and limitation of movement. Arthroplasty may be achieved by the excision of bone ends with or without interposition of flexible material, by the grafting of articular cartilage and by total or partial prosthetic replacement

The Hand,Patients with rheumatoid disease will tolerate gross hand dysfunction because the disease progresses at a rate which allows them to accommodate to their disability, and apart from epi-sodes of acute synovitis, pain is not a prominent feature. Patients nevertheless will often attend early for a disfiguring rheumatoid nodule (Fig. 14.1).

Arthrodesis,torily, whereas in rheumatoid disease, where involvement of nearby joints will often progress, it may cause severe disability. In the lower limb an arthrodesed joint may become an insuperable barrier to rising from a chair and in the upper limb it may inactivate an otherwise useful hand.

The Shoulder,lder is unusually the site of severe destruction and then almost exclusively when there is severe destruction of other joints. Destruction tends to appear late in the shoulder. This may explain why most surgeons have been hesitant in the past to operate on rheumatoid shoulders.
