Abstract
As for those patients indicated with carotid surgery, antiplatelet therapy should be initiated before procedure. Currently, the medication algorithm may be various at different centers and authors only give Aspirin. For adult patients, the baseline dosage of pill is 300 mg per day lasting 3 days. If a patient has taken Aspirin (100 mg per day) longer than 1 month, the same oral dosage should be kept until the day of CEA. However, it is recommended to increase the dosage to 300 mg for 3 days prior to surgery if patients with less than 1 month history of single antiplatelet therapy. If available, thromboelastography may be used to detect the effect of anticoagulation in each individual.

Abstract
Case 1: A 59-year-old gentlemen presented with 1 month history of dizziness with one time falling down when he got up in the morning. Physical examination (PE) was uneventful. CTA demonstrated bilateral carotid stenosis (Lt: 50%; Rt: 90%) (Figs. 1 and 2).

Abstract
The prior three chapters aimed at describing common knowledge points of CEA and tried to make such technique popularized for readers. In the clinical practice, however, each case or patient was specific and miscellaneous situations may encounter which are rarely discussed among above contents. In this chapter, a majority of particular issues that approached previously by the authors will be presented and discussed with valuable comments.
