描述.Intensive care is a rapidly changing area of medicine, and after four years from the 2.nd. edition the volume editors and authors have deemed necessary to update it. In the recent years, in fact, five new randomised controlled trials and five new meta-analyses demonstrate that selective decontamination of the digestive tract [SDD] is an antimicrobial prophylaxis to prevent severe infections of not only lower airways but also of blood. Additionally, SDD has been shown to reduce inflammation including multiple organ failure and mortality. An intriguing observation is the evidence that SDD using parenteral and enteral antimicrobials reduces rather than increases antimicrobial resistance. Moreover, a new chapter on microcirculation had been added. .The volume will be an invaluable tool for all those requiring in depth knowledge in the ever expanding field of infection control..

Classification of ICU Infectionshave been accepted to distinguish community- from hospital-acquired infections, including ICU-acquired infections. However, many clinicians appreciate that an infection due to a microorganism carried by the patient on admission to the ICU and that develops 48 h of ICU stay cannot be considered as being a “true” ICU-acquired infection.

Outbreaks of Infection in the ICU: What’s up at the Beginning of the Twenty-First Century? are the two known to cause outbreaks. Secondary endogenous infections can be controlled by enterally administered antimicrobials and should be integrated into the routine infection control measures. Exogenous infections can be controlled by topically applied antimicrobials and hygiene.

Preventing Infection Using Selective Decontamination of the Digestive Tractn critically ill patients. The purpose of SDD is to prevent—or eradicate if initially present—the oropharyngeal and intestinal abnormal carrier state of potentially pathogenic microorganisms, mainly aerobic Gram-negative microorganisms, but also methicillin-sensitive . (MSSA), and yeasts, leaving the indigenous flora predominately undisturbed.
