描述Considerable impetus was given to the study and understanding of cere brovascular anatomy by Thomas Willis and his contemporaries in the seventeenth century, yet almost two hundred years were to pass before further significant advances were made in this field. Then, from the mid nineteenth century onwards, the dark ages of cerebrovascular research gradually lifted through the efforts of such workers as Luschka, Heubner, and Windle, whose pioneering anatomical studies formed the basis of the present-day understanding of the morphology of the cerebral circulation. The turn of the century saw an increasing influence of the early neurolo gists in describing anatomy of cerebral vessels in relation to their areas of distribution and to the production of focal deficits through specific vascu lar lesions and anomalies. Later still, Padget and others made important observations concerning phylogenetic and developmental aspects of the cerebral circulation. These anatomical and clinical studies were remarkable enough but the real breakthrough in investigating cerebral pathophysiology and in devis ing appropriate corrective neurosurgical procedures had to await the re markable advances i

The Vascular Anatomy of the Cerebral Hemispheres, ent with cerebrovascular disease. The regional blood supply to the cerebral cortex, basal ganglia, brain stem, and cerebellum serves as a background for drawing neuroradiologic-pathologic correlations. In conjunction with the computer tomography (CT) scan, the regional angiographic approach represen

Vascular Anatomy of the Posterior Fossa,or carotid angiography. Though vertebrobasilar angiography continues to be an integral part of patient management, its indications are now limited to (1) the evaluation of vascular lesions, especially in the clinical setting of subarachnoid hemorrhage, (2) the evaluation of vascular posterior fossa

Hematologic Considerations in Cerebrovascular Surgery, ter provides the basis for understanding the pathologic processes involved in cerebral thromboembolism and its treatment with antiplatelet aggregating agents and anticoagulants. It establishes the rationale for antifibrinolytic treatment of aneurysms and provides an insight into the assessment of th

Cardiovascular Considerations in Cerebrovascular Surgery, cerebrovascular surgery. Cardiovascular disease is an important risk factor in the management of occlusive cerebrovascular lesions, aneurysms, and hypertensive hemorrhage. This chapter describes the evaluation and management of patients with cardiac disease in anticipation of cerebrovascular surger

The Operating Microscope in Microvascular Surgery, surgery to develop in three fundamental ways. First, observations made with the microscope have made us cognizant of anatomical structures that were previously taken for granted. The complex anatomy of perforating arterial branches are now appreciated because of observations made with the operating
